For Your First Appointment

Please plan to arrive fifteen minutes before your appointment time. We will ask you to fill out registration paperwork

To your first appointment, please bring:

• Your referring doctor’s information, including name, address and phone number
• Your current insurance card
• Photo ID

You may bring up to 2 people with you to this informative session. Any children must be accompanied by an adult who can care for the child during your examination.

Cancellation policy

If you must cancel an appointment, please notify us as soon as possible. We require 24-hour notice to consider an appointment canceled.

As a courtesy, our receptionists confirm all appointments 24-48 hours in advance, but you are still responsible for remembering your appointment date and time.
